As a Social Purpose Corporation, Consistent Care Services is able to focus the company's efforts on improving the health of the communities the company serves by coordinating the care of patients. The company's primary goal is to connect the company's patients to the resources they need with a secondary goal of reducing inappropriate utilization of... health care services. By helping the company's patients access local services the company can help them manage their health needs in a more sustainable way.
