We are an HMO provider through the UCLA Managed Care Health Systems. You will need an authorization and authorization number from your UCLA primary care physician and/or UCLA medical utilization review board. This authorization must be obtained before your first visit. If you do not have this authorization, you will be responsible for the total cha... rges incurred or services rendered. Please speak to your Doctor if you are interested in beginning physical therapy. Co-payments must be paid at each visit. As a courtesy for our patients we will contact your insurance carrier to determine your benefits for physical therapy and submit all claims to your carrier, taking care of the billing for patient convenience. Call us directly to inquire about your individual medical insurance coverage. For those uninsured clients, WPTR works with the individual to achieve an affordable cash payment for their physical therapy services.Read More
