Transitional Living Centres (TLC) was founded in 1999 to provide community based residential rehabilitation services for persons with an acquired brain injury. Our organization believes in promoting independent living skills which will allow the individual to achieve the highest possible level of participation in the community. Our on site staff ar... e available 24 hours per day to provide assistance to our clients. Community outings are supported by staff on a one-to-one or small group basis as required. Each client has an individualized program designed to assist them in achieving their social, vocational and community reintegration goals. We promote holistic rehabilitation involving cognitive, physical, emotional and spiritual components. TLC provides a flexible comprehensive program which balances the need for structure with increasing levels of responsibility according to the abilities and goals of each individual. Our trained Community Reintegration Workers are supported by Primary Workers and Social Workers/Case Managers who develop and implement the rehabilitation plan.
