Call the office and provide all the information needed along with a prescription from your pediatrician, to initiate auth... orization for services. For out of network clients we charge competitive & affordables rates for our services. We will provide you with documentation & invoice with procedure and diagnosis codes to submit a claim to your insurance company. Please contact your insurance company prior to scheduling an evaluation or therapy for out-of-network benefits. See if a physician prescription is required for your child to receive therapy evaluation or services. We accept checks & cash only for all co-pays. We accept credit cards for all other services.
