Established in 1855. Sutter SeniorCare PACE is a nonprofit health program exclusively for frail, older adults. The program is committed to delivering high-quality, comprehensive care services to our participants, anticipating problems, and preventing or avoiding the need for premature nursing home placement. Because Sutter SeniorCare PACE focuses o... n complete, coordinated care, participants receive a broad range of services, including all benefits provided by Medicare and Medi-Cal and more. We offer medical care (primary care physician, nursing home care, laboratory, medication and ambulance service), adult day care (food, social services, personal care and transportation) and in-home health care and support (nursing care, homemaker services and delivered meals). Most care is provided by an interdisciplinary team of physicians, nurses, social workers, rehabilitation therapists and others.
