It is Our Mission to assist patients and their families with the inevitable symptoms of a terminal illness. Our goal is to improve the quality of a patients remaining life and to offer comfort and dignity in the process. Hospice is not a place, it is a form of alternative care when a person has decided to no longer seek curative treatment. (Curativ... e treatment includes common therapies like chemotherapy, blood transfusion, dialysis etc.) When the decision is made with the physician to stop curative treatment, a hospice team will come to your place of residence and provide care, and also help with the patient's physical, emotional, and spiritual comfort. This team will remain with the patient offering this support throughout their hospice tenure. The PCH care team consists of physicians, nurses, certified home health aides, chaplains, social workers, and counselors. Each of these professionals is dedicated to helping patients and families prepare for, understand, and make informed decisions when facing a life limiting illness. We seek to understand the unique physical, emotional and spiritual needs of each of our patients and their families. It is from that knowledge that we can provide the compassionate, individualized support you need. Our care is provided 24 hours a day, 7 days a week, wherever you live- at home, in an assisted living or skilled nursing community.
