There must exist a normal inability to leave home; Leaving home must require a considerable and taxing effort. 1) Patient demographic sheet listing name, address, phone number and insurance information. 2) Written order for Home Health Care services from a physician and/or nurse practitioner. RN/PT to evaluate and treat because/for: Diagnosis and r... eason for home health services. 3) Recent office visit note or progress note (Face to Face Encounter) which includes patients history and physical along with a current medication list. 4) Home Health Certification Statement signed, credentialed and dated by physician and/or nurse practitioner. (Date used on Certification Statement for Face to Face encounter must be the same date as visit/progress note provided) 5) Name and phone number of physician and/or nurse practitioner who will follow patient in the community and sign Plan of Care and home health orders. Please fax information to (630) 654-5705. Once we receive the information requested, we will do medical and financial clearance and check staffing availability. The goal of Phoenix Home Cares clinical team, under the guidance of a physician, is to develop a plan of care to treat each patients medical condition. The plan of care focuses on the patient which includes the family, and/or caregivers, and is supported by the Phoenix Home Care team. The plan of care includes assessment, goal setting, treatment and discharge. Phoenix Home Care improves patient outcomes by the use of evidence based practice to ensure the quality of care provided. At the Physicians order, a Phoenix case manager will conduct a patient phone consultation. During this consultation, the case manager will speak with the patient, family, and/or caregiver to plan a successful transition to Phoenix Home Cares services. A Registered Nurse, Physical, Occupational, and/or Speech Therapist will perform a detailed assessment of the patient at his/her home environment.
