A medallion guarantee stamp may be obtained from a broker-dealer, wealth management firm, some Canadian banks and most US banks, savings associations, or other financial institutions which participate in a recognized Medallion program such as STAMP, SEMP or MSP. Alternatively, a signature guarantee stamp is accepted from Bank of Nova Scotia, RBC or TD Bank. Firms providing a guarantee will require an existing relationship. In order to complete a transfer, the following documents are required. Original Securities Transfer Form. Complete all required fields, authorized officer(s) or partner must sign and obtain a signature or medallion guarantee. If you have obtained a Signature Guarantee instead of a Medallion Guarantee, an original or certified copy of the Corporate Resolution, Resolution of Sole Director or Partnership Agreement is required to provide evidence that the signatory has authority to sign on behalf of the Corporation or Partnership. Resolutions must be dated within 6 months. Letter of Instruction - Detailing the transfer request, delivery instructions and contact information.
