The ISC for ME, LLC team supports people to identify their medical, employment, financial, social, educational, and other goals, identify the services necessary to meet those needs including natural supports in the community and waiver providers, learn how to advocate for their goals and facilitate access to identified services to live as independe... ntly as possible in their community.
