Horizon House

Horizon House is a Community-Based Residential Facility located in Wautoma, Wi. Our home can accommodate up to eight residents. We use evidence-based practices with the aim of preventing long-term re-hospitalization and re-institutionalization. The physical and clinical programs are both designed to promote a least restrictive environment; to promote resident's mental, physical and spiritual well-being; to provide them an opportunity to realize their full human potential; and to aid residents with their long-term recovery and reintegration into the community. Our goal is to empower each of our residents to live a life that is active, productive, fulfilled and happy. Horizon House is centrally located near town and close to several great ammenities; including medical facilities, restaurants, and shopping. All while still sitting on the outskirts of town, offering our residents the privacy of a country setting. Each resident will have their own private room. They will have access to the dining room, living room, kitchen, recreation room and a large fenced in yard that includes additional buildings for activities. Horizon House is professionally staffed twenty-four hours per day, 365 days per year. Our staff's main goal is to help each resident learn the necessary skills to gain back their independence, with the hope that they will be able to live on their own with little to no assistance.
