Help Me Grow

Raising a child is hard work that can bring up a lot of questions for parents and caregivers. We believe that children thrive when families know where to turn with their challenges and concerns, and when they receive support that aligns with their values and priorities. Our goal is to connect families quickly and easily to the information and resou... rces they need to support their childs growth. Providers who work with children and families can also use support. Through Help Me Grow, they can access resources such as screening tools, referral recommendations, and care coordination assistance. They can also refer parents and caregivers directly to Help Me Grow and feel confident that a family will be connected to supports and resources. Rather than functioning as a stand-alone program, Help Me Grow serves as the glue to connect families with providers who are able to provide supports and services for children and their families in San Mateo County. Help Me Grow San Mateo County is a part of the national Help Me Grow network , a growing coalition of states, communities and individuals working together to improve early childhood systems. Four core functions characterize the Help Me Grow model: Centralized Access Point: The Centralized Access Point (call center) assists families and providers in connecting children to appropriate community-based programs and services. Family & Community Outreach: Family & Community Outreach promotes Help Me Grow, facilitates provider networking and promotes childrens healthy development. Child Health Care Provider Outreach: Child Health Care Provider Outreach promotes universal screening and supports early detection and intervention efforts by medical providers.
