Our network includes the area's leading healthcare facilities - Arroyo Grande Community Hospital, French Hospital Medical Center and Marian Medical Center. We offer a free Medication Consultation Program... giving our members an opportunity to meet one-on-one with our pharmacist who will review medications, provide information and make recommendations for effectiveness and possible cost savings. We also offer our members a free Wellness Program with classes and workshops to help them stay healthy. Physicians Choice Medicare Plus HMO is offered by GEMCare Health Plan, Inc., a licensed healthcare service plan with the State of California and contracted with the Centers for Medicare and Medicaid Services to provide Medicare benefits to beneficiaries in Kern County and zip codes in San Luis Obispo, Santa Barbara and Ventura counties. Physicians Choice Medicare Plus HMO is offered by GEMCare Health Plan, Inc., a Medicare Advantage organization with a Medicare contract, and is available to those who are entitled to Medicare Part A and enrolled in Part B.
