America's Support Coordination and Management is a service supports coordination agency "helping you to meet your needs and empower independence" in the community of your choice. We provide excellent quality service to individuals regardless of their disability and/or age. We are a key liaison to ensure that our participants live independently, saf... ely and comfortably in the community of their choice with the needed services identified on their Individualized Service Plans. It has always been our goal to help individuals integrate into their own communities. We give them tools to achieve the highest level of independence possible and will guide them towards living a quality life. There are many community resources that can be tapped into when helping these individuals. America's Support Coordination and Management is part of a vast network that includes such community resources.
