The scope of our population served are intended to cover all eligible Pennsylvanians 18 years of age and older based on our current spectrum of accepted waivers. Our team of trained Supports C... oordinators provide individuals with the necessary tools and resources to help them achieve the highest level of independence possible, with an emphasis on remaining at home comfortably and holistically. Supports Coordination is a vital part of accessing Home and Community Based Waiver Services. Supports Coordinators (SC) are responsible for connecting to resources, assessing service needs, supporting independent living goals, monitoring waiver services and advocating whenever possible to get your needs met. The ease of coordinating supports and resources to help find home health care attendants, install wheelchair lifts/ramps, access extending health services and therapies is dependant upon how well trained, responsive and knowledgeable the Supports Coordination Agency and their staff are.
