We want to get some of the people who are closer to the cutting edge to come together and share their experiences and maybe come up with an enhanced model and a strategy for how it could work and who would be responsible for promoting and moving it along , Enson explains.Although CSHP has agreed to take the lead on this , the key is for us to be cooperative with CPhA [ the Canadian Pharmacists Association ].You cannot provide seamless care from one side of the gap..( At press time , the task force was planning a teleconference on seamless care with CPhA staff to take place in late November. ).
A seamless care prototype , he adds , would need to address not only how hospital pharmacists might transfer information to community pharmacists but also how they might communicate with pharmacists at other practice sites , such as residential care facilities and health care clinics.
