The number one way for a physician to get the full benefit of an HRA is to use it as a diagnostic tool, says David M. Paperny, MD, director of adolescent services for Kaiser Permanente's Hawaii region. Paperny developed a multimedia system called the Youth Health Program that asks patients a series of questions — including follow-up questions based on their answers — and then plays videos that address the risk factors that the software identifies. The system also prioritizes the patient's problems and prints them out for Paperny to review before seeing the patient.

    600 questions

"There's a question selector, so you can set the questions to 'comprehensive,' which can give about 600 questions," he says. Options also include narrowing the questions down to drug risk assessments, affective disorders, and other categories. Paperny has even replaced his patients' annual physicals with a visit to the computer and a half-hour session with a health educator who counsels teenagers about risk factors. Physical exams are given if needed, he says.

"The system prioritizes problems, and that's really more important than just giving standard advice," he says.
