    That's not a problem, individually, says Dr. David Keith, but it is what he calls an ecological problem.

    "It interferes with how we handle the issues of freedom and responsibility," he says."It affects the feelings of responsibility for children."

    Keith is the chief of child psychiatry at Syracuse's SUNY Upstate Medical University and one of the editors of the new text, "Family Therapy as an Alternative to Medication: An Appraisal of Pharmland."

    He says clinical judgment has faded because of the use of medication in mental health treatment.He sees this sometimes with parents who seek help with a difficult child.They may first see a therapist who advises they're not engaged enough in their child's life.They don't like what they hear, so they find another therapist who tells them the child may have an attention deficit - and prescribes medication.

    Keith isn't entirely opposed to medication.He says it's "sometimes necessary but rarely sufficient" in therapy.

    During his career as a psychiatrist, he's watched the evolution of health maintenance organizations and the powerful marketing of pharmaceuticals directly to the public.
