Cheryl Kahn, a spokeswoman for New Orleans-based The Oath, said hospitals and doctors are free to make contracts with different health care companies involving different types of plans.

She said a lot of changes are occurring this year.

"There's a lot of things changing with the different insurance programs. We are also making some changes too," she said.

Kahn and hospital officials noted that The Oath's other HMO products are still accepted at the hospitals.

Officials said SmartPlan 65 members receive added benefits to traditional Medicare, such as prescription drug coverage and vision benefits.

Kahn said the problems with the hospitals, and decisions by The Oath to reduce coverage in some plans, have stemmed from the federal government's decision to reduce reimbursements to insurance providers, hospitals and skilled nursing facilities.

However, she said, The Oath has provided customers with notices about the changes.

Cheryl Kahn , a spokeswoman for The Oath , said the medical plan is experiencing the same problems as the hospitals.

( The hospitals ) need more money from us and we can't afford to pay more , she said. The reason goes back to the Medicare reimbursements we receive are not adequate to cover the rising costs of health care. So basically we're in the same position..

Kahn and Hospital officials stressed The Oath's other HMO products are still accepted at the hospitals and SmartPlan 65 members can continue to see network doctors at Thibodaux Regional and Terrebonne General.

Members can still go to the hospitals and SmartPlan 65 will pay the hospital the Medicare allowable , Kahn said.

SmartPlan 65 members also receive added benefits to traditional Medicare , such as prescription drug coverage and vision benefits , Kahn said.

The trend away from Medicare health maintenance organizations is not a local phenomenon. In 1997 there were eight Medicare HMOs operating in Louisiana. Now there are only three. The remaining Medicare HMOs include Ochsner , Tenet and SmartPlan 65. The reduction of plans is a result of the federal government's decision to reduce reimbursements to insurance providers , hospitals and skilled nursing facilities.
