   Dr. Collins was born in Los Angeles, California. He received a B.S. in Biology from the University of California at Los Angeles in 1984. Following this, he attended the University of Southern California School of Medicine where he received his MD in 1989.
   
   His professional training continued at the University of Cincinnati Medical Center in the Department of Physical Medicine & Rehabilitation in Cincinnati, Ohio, where he completed his residency in 1993.
   
   He holds medical licenses in both Ohio and Arkansas and in 1994 was Board Certified by the American Board of Physical Medicine & Rehabilitation. He is a member of several professional organizations including the American Congress of Physical Medicine & Rehabilitation, the American Academy of Physical Medicine & Rehabilitation, and both the Arkansas and Pulaski County Medical Societies.
