   Director of Social Services: Michele Chawla, LSW
   
   Michele is a graduate of Elms College with a Bachelors Degree in Social Work. She has over ten years experience in long term care, the majority of that experience has been at Renaissance Manor on Cabot. Michele is well versed in patient rights, the formulation of Advanced Directives, and the mechanics of how to apply for and use Medicare and Medicaid benefits. She is actively involved with our residents and families from the time of admission up to one month after discharge. She also works closely with the nursing, rehab, and activities departments to ensure that each resident is receiving appropriate care and services in order to maintain optimal psychosocial well-being. Michele is also responsible for discharge planning and makes referrals for home services so that our residents who return home receive appropriate help.
