   In other states, managed care led to dramatic improvements in mental-health care, but only through the cooperation of caregivers and Medicaid managers.
   
   What Nebraska set in motion was anything but cooperative. Pressed by the Legislature, the state gave ValueOptions a pool of money, the discretion to spend it and the mission of trimming the state's costs by 25 percent.
