"Traumatic brain injury is the signature injury of this war," said Dr. Shane McNamee. He's Medical Director of Polytrauma at McGuire. He says 62 percent of the troops injured in Iraq suffered traumatic brain injury. But that doesn't tell the whole story.
"What we're actually more concerned about is the individuals who suffer one, two, five, six, ten mild brain injuries who are going undiagnosed," he said.

I discussed one of these cases with Dr. Shane McNamee, medical director for polytrauma at the Hunter Holmes McGuire VA Medical Center in Richmond, Va. A soldier was under McNamee's care for eight or nine months; he was horribly wounded with both legs blown off, a shattered pelvis, a deep arm gash, an open abdomen, severe head wounds, spinal cord injuries and acute anxiety. "One of the most gruesome things I've seen in medicine,'' McNamee said.
This case had a good outcome, McNamee said. After seven months of intensive care and rehabilitation, the soldier was transferred home, where he has daily visits from medical and rehab specialists and is learning to dress and feed himself. But he will not advance much beyond that ability, doctors said.
"How do you fix a guy like that? You can't,'' McNamee said.

Seeking treatment at a polytrauma rehabilitation center rather than a more general VA hospital is especially important for service members with severe injuries, said Dr. Shane McNamee, the chief of physical medicine and rehabilitation service at the Hunter Holmes McGuire Medical Center in Richmond, Va.
"Somebody who has sustained multiple blasts and has severe psychological problems, or a severe blast and has multiple amputations and traumatic brain injury and all the things associated with that, they're going to do a lot better at a polytrauma rehab center than somewhere else," McNamee said.
