"I've been talking to people here," said Richard Averbuch, senior director of media communications for the MHA, "and the kind of losses we are looking at are not like anything that has been seen since the 1970's when they started collecting the data." While most experts agreed that low reimbursements by the three major health care payers - Medicare, Medicaid, and private insurers - are primarily to blame for the current fiscal situation, there are varying opinions on which payer is the biggest cause of the problem. Averbuch pointed to Medicaid's reimbursing an average of 71 cents on $1 for services. "That amounts to underpayments of $200 million a year," he said. Averbuch on the other hand, felt that the teaching hospitals, because they receive funding from the federal government and thereby a reduced rate from Medicaid, are more likely to be in trouble. In the past decade, according to Averbuch, the state has lost 30 percent of medical beds with the closing of 25 percent of its hospitals. And, as Averbuch pointed out, there is not a lot of room for error in the system.When one hospital is floundering, there are not enough beds available for others to pick up the slack. Even when a hospital has a positive operating budget, that doesn't guarantee it will remain positive the next year and doesn't necessarily mean that the hospital is doing well.In order to reinvest and upgrade equipment and facilities, a hospital needs to make more than a one or two percent yearly profit, Averbuch added. The future looks grim unless changes are made."We have to address these issues," said Averbuch.
